*20 Skilled Physical Therapy Sessions per year ($2000 value)

*After care follow up, medically based training/coaching (Up to 2, 60 minute sessions a month; $1200 value)

*Two 60 minute massage sessions (per year) ($120 value)

*Access to providers via E-mail and telephone

*Home visits if needed (certain exclusions apply; call for details)

*Home or office ergonomic assessment (one per year; $200 value)
